Before you begin
Editor, Admin, or Owner. Choose Form for structured responses or Evidence request when a reviewer needs proof. For formal learner completion, add the published tool to an assigned Guided Journey. Supported embedded article tools can collect separate optional responses; direct standalone pages are unsaved previews.
1. Build the request
Open Learning Studio and choose Form or Evidence request.
For Form, enter the name, description, knowledgebase, and instructions. For Evidence request, complete Request name, What must be demonstrated?, and Acceptance criteria before continuing.
Add and order the fields. Use concise labels and mark only essential fields Required.
For evidence, specify the proof required, such as a photo or file plus supporting observations.
2. Configure review and placement
Review Responses or Review rules. Set identification, save-and-resume, confirmation, and review requirements as offered.
Choose the placement: standalone, journey, or article. Check the destination and whether completion is required.
Review and save the draft, then publish it from the library.
Add the published form to a Guided Journey, publish and assign the journey, then test a submission from that Reader assignment. Where review is required, check the submitted work and record the reviewer outcome.
Supported reusable forms embedded in eligible articles can collect optional article responses, including anonymous responses when offered. Review these separately in Learning Studio → Responses. Public article forms with file-upload fields cannot be submitted there; use a signed-in assigned journey when file evidence or formal completion is required.
Check your result
Learners can submit the requested information, and reviewers can find the resulting records.
If something goes wrong
Missing upload: check the accepted file requirements and upload result.
Submitted but incomplete: a required review may still be pending; inspect the record before asking the learner to submit again.
